Ending an appointment and checking the note

5 min readFor clinical staff

When the visit is over, press End. Rebrief stops transcribing, finishes the note from the whole appointment, and checks it. Then you read it over and get it into your chart.

Press End

End sits in the status line at the top centre of the appointment page. On a phone, it's in the bar at the bottom of the screen.

From another page, use End on the Scribe card in the bottom right corner, or click the Scribe row in the left rail.

A soft chime plays. Transcribing stops, and the note and the chart are saved.

While Rebrief finishes the note

  1. The status line reads Refining note. Right after End, it may also show how much of the last stretch of audio is saved, such as 62% saved.
  2. The Note tab shows a saffron spinner.
  3. When the note is final, the status line reads Note ready, with a check. It stays until you move the mouse, click or type.
  4. After that, it reads Ended and the time.

On a phone, the bar at the bottom shows Refining note… while this happens.

The edit tools wait for Note ready, and so does Export.

What End does to the note

  • If Rebrief refined the note during the appointment, End reads the last part and makes one more careful pass.
  • If not, End transcribes the whole appointment again, more carefully, and rewrites the note from both transcripts.

Either way:

  • Anything you typed stays as you wrote it.
  • Clinical facts the conversation states, but the note missed, are added.
  • Misheard words and numbers are fixed.
  • Statements the conversation doesn't support are taken out, such as another person's story written as the patient's.
  • Tooth numbers stay as they were said. A baby-tooth number appears only when someone named a baby tooth or the patient is a child.
  • X-ray readings, such as caries grades, go under Radiographic findings.

Words that changed take a saffron highlight, which fades once you've seen it. Undo on the note's bar takes back a change.

If End can't improve the note safely, it keeps the note as it was. On a phone, the bar then reads Note kept.

What End checks

  • The medicolegal check. A chip on the note's bar reads Checking for medicolegal omissions…, then a result such as 2 medicolegal omissions to check or No medicolegal omissions found. A missing detail shows as a faded line in the note, with Add and Skip. A line whose findings and conclusion disagree gets a red underline, with Dismiss. So does a baby-tooth number nobody said. Click the chip to go to the first one.
  • Sources. A chip such as 12 sources counts the sentences Rebrief linked to what was said. Click in a sentence, then select Source to see the words.
  • The chart. Rebrief reads the transcript again and corrects the tooth chart and the perio chart. A short notice says when it changed something.
  • Tasks. Rebrief writes the appointment's to-dos from the note, in the Tasks tab.

What opens by itself

+ Create opens once, in case the visit needs a document or a form. To stop that, switch on Don't open this at End at the foot of the menu.

When Rebrief has questions about the note, a chat opens on Polish your note. It asks about the few details a note needs that nobody says aloud, such as the anesthetic, the shade or which X-rays were taken.

  1. Pick an answer by its letter. Some questions take more than one.
  2. Or select Something else and type it as it should read in the note.
  3. Select Next, or Skip to leave it out. The last question has Done.

Each answer is written into the note at once, highlighted. Undo puts it back. When the questions are done, describe any other change in plain words, or pick one of the suggested edits.

If the note already has what it usually needs, the chat doesn't open. To stop it opening at End, switch on Don't open this at End at the foot of the chat.

Both switches are also in Settings › Notes & dictation, under When an appointment ends: Polish the note in the chat and Open + Create. They apply on this computer.

The suggested question chips beside the prompt bar stay after End, until you leave the appointment page.

Check the note

  • Read it top to bottom. Use Source on any sentence you want to check against the conversation.
  • Type straight into the note. What you type is kept as you wrote it.
  • Highlight some words, click the small bar that appears, and type what to change.
  • Or ask in the prompt bar, such as "shorten the plan in the note". The change is highlighted, saved, and can be undone.

Get the note into your chart

Use Export at the end of the tab row.

  • Copy text puts the note on your clipboard. Paste it into your practice software.
  • PDF on letterhead downloads the note as a PDF.
  • Under Odontogram, Copy image and PDF take out the chart.

Clinics connect their practice software to Rebrief through the Windows app, the Rebrief app installed on a clinic computer. In the Windows app, on plans that connect to practice software, Export can also send the note and the chart into it. The send runs as a card in a chat, with a Stop button. You can also ask the prompt bar to send the note.

Before End, Export says "End the appointment first — the note is still being written." While End finishes, it says "The note is being polished — one moment."

Press Resume on the same appointment, at any time. Rebrief carries on in the same note, and the next End finishes it again.
